Purpose

To furnish the Department of Labor (DOL) with a semi-annual report on compliance with and enforcement of the Davis-Bacon and related Acts. Click here for information on the Davis-Bacon and related Acts.

Coverage

Report all permanent full- and part-time employees filling a position on the Subcontractor’s approved staffing plan and whose positions are under the Davis-Bacon and related Acts. If no positions are under the David-Bacon and related Acts, the report is not necessary.

Frequency

Semi-Annually (FY) – Reporting period ends on March 31 and September 30

Due Date

Subcontractors must submit the report by the 10th day following the close of the reporting period. This report should be submitted to:

Instructions
Questions 1 through 10 – Answer questions 1 through 10 as instructed below.
  1. Question 1 – Choose the correct reporting period from the pull down list on the report form.
  1. Question 2 – Enter the number of your company’s subcontractors.
  1. Question 3 – Enter the total dollar amount of the subcontracts identified in Question 2.
  1. Question 4 – Enter the number of subcontracts mentioned in Questions 2 against whom complaints were received.
  1. Question 5 – Enter the number of investigations completed.
  1. Question 6 – Enter the number of subcontractors found in violation.
  1. Question 7 – Enter the amount of back wages found due in both a) Davis-Bacon Act, which involves prevailing wage violations, or b) Contract work Hours and Safety Standards Act, which involves overtime violations.
  1. Question 8 – Enter the amount of back wages paid for both a) Davis-Bacon Act and b) Contract Work Hours and Safety Standards Act.
  1. Question 9 – Enter the total number of employees paid wage restitution under the Acts mentioned in item #7 and item #8.
  1. Question 10 – Enter the amount of liquidated damages assessed under Contract Work Hours and Safety Standards Act.
